Front - "Tempe Lodge No. 15, F. & A.M. / Tempe, Arizona / No. 4 / This is to certify that brother / Carl T. Hayden / Whose name is signed on the margin hereof / is a member of this lodge / (seal of); having paid his dues through / December, 1967 / Ben C. Morrow / 1967; Secretary / Void unless signed by member." Along left side of front is Hayden's signature. Back - "Grand Secretary's Certificate / The Grand Lodge of / Free and Accepted Masons / of the State of Arizona / This is to certify … " etc.Status
