DescriptionThe Osteopathic Physician and Surgeon's Certificate #128 Jan. 1, 1980 for Arizona, seal and all is mounted on wood and shellaced over instead of framed in glass. It was issued by the Arizona State Osteopathic Board and signed by Newton McBridge and A.B. Stanio. On the back are pasted two bits of paper. 1. County recorder's filing record 2-21-50 and 2. another certification of record with Maricopa Country in Glendale, aug. 1950. Both have docket numbers, seal and signatures.Status
