DescriptionThis shield-shaped medium brown plaque has a two-colored long metal piece with scrolled outward ends. There's a double 3-D metal palm leaf at the bottom of the wood. The plaque says, "This is to name Leonard Ellis Staff Jr. D.O. as general practitioner of the year 1971 for distinguished service to both his patients and his profession"; "Presented with gratitude and affection by the Arizona Society of the American College of General Practitioners in Osteopathic Medicine and Surgery," "May 1971." It has a hook on the back.
